Case Coordinator- Stabilization

As a Crisis Case Coordinator, you'll play a critical role in providing immediate assistance, crisis intervention, and ongoing support to individuals in need. This rewarding position combines compassionate care, problem-solving, and case management to help clients connect with the resources and services they need to achieve stability and recovery.

To support a safe and healthy environment for the individuals we serve and for one another, all employees of Radiant Health are required to comply with organizational health and infection prevention requirements as a condition of employment. • Bachelor’s degree in social work, psychology, counseling, or a related field.
• Minimum of 2 years of experience in crisis intervention, case management, or a related field. Experience working in rapid response or emergency services preferred.
• Relevant licensure or certification preferred.
• Valid driver’s license and reliable transportation for community-based work.
• Ability to work flexible hours, including evenings, weekends, and on-call shifts as needed.
