Create relatable content, measure performance, sustain community and partner relationships, and support promotion and execution of fundraising and organizational events.

The Case Coordinator provides comprehensive case management and support for individuals with mental health or substance use disorders, promoting recovery and well-being.

Provide compassionate clinical support to mental and behavioral health clients: obtain vitals, urine drug screens, assist with orders, transcribe medication orders, coordinate prescriptions/prior authorizations, administer injectables, follow up on labs, document accurately, and maintain compliance and a safe clinical environment.

The Peer Support Specialist provides compassionate support and peer counseling to individuals facing mental health or substance use challenges, using personal experiences to guide and encourage them on their recovery journey.

Provide immediate crisis intervention and case management to individuals experiencing mental health and addiction crises. Perform assessments, connect clients to community resources, offer ongoing support, document care, and work flexible hours including evenings, weekends, and on-call shifts. Maintain required health and immunization documentation and adhere to infection prevention protocols.

Use the evidence-based Supported Employment model to help clients with mental health challenges identify career goals, build resumes, secure competitive jobs, provide job coaching and skills training, develop employer relationships, attend networking events, educate employers, offer case management, and coordinate with clients' therapists and treatment teams.

The RN will provide nursing care, assess client needs, monitor medications, and support clients with mental illness and substance use in a community setting.
