Title: Care Coordinator

We’re hiring a full-time remote Care Coordinator to be the first voice a patient – or their family -hears when reaching out to Sailor Health. In this role, you’ll guide older adults and their caregivers through the intake process with warmth, clarity, and professionalism, ensuring they feel supported every step of the way.

This role is a unique blend of patient support, care coordination, and emotional guidance. You’ll be the steady hand helping each new patient navigate the journey into care, from matching with the right therapist to feeling confident and understood every step of the way. It’s a perfect fit for someone who thrives at the intersection of empathy and execution, and who finds meaning in turning a moment of vulnerability into one of reassurance and trust.

This role is especially well-suited for someone with an interest in mental health and healthcare access – since you’ll often be the first emotional point of contact for individuals seeking care.

Who You Are
  • Empathetic and clear communicator. You speak with kindness, listen actively, and make others feel at ease – even during difficult or emotional conversations.

  • Highly organized and detail-oriented. You can juggle multiple patients at different stages in the process and never let things slip through the cracks.

  • Tech-comfortable. You’re comfortable learning tools like Google Workspace, EHR platforms (e.g., Healthie), or CRMs like HubSpot, and pick up new software quickly.

  • Mission-aligned. You care deeply about improving care for older adults and are excited to play a key role in someone’s first step toward better mental health.

  • Experience welcome, not required. You may have worked in healthcare, customer service, or an administrative support role before – but most importantly, you’re organized, compassionate, and eager to grow.

Why Join Sailor Health
  • Mission with impact. Help bring life-changing care to a population that’s too often overlooked.

  • Remote-first team. Enjoy the flexibility of remote work while staying closely connected with a thoughtful, collaborative team rooted in purpose.

  • Growth and ownership. Be part of a small, agile team where you’ll take initiative, shape key processes, and grow as we grow.

  • Make someone’s day – every day. Your work helps older adults and their families feel seen, supported, and cared for.

If this sounds like you, we’d love to connect. Join us in redefining what it means to age with dignity, connection, and mental wellness.

What We Do

Sailor Health is on a mission to solve the mental health crisis among older adults. Getting older in America is tough: 90% of seniors live with chronic health conditions. 40% report feeling isolated. And despite having the highest suicide rates of any age group, older adults receive mental health therapy 70% less than the average American. Sailor Health is building the AI-native health system for happier and healthier aging. Sailor offers virtual mental health, care navigation, and AI care agents, all covered by Medicare. We've built the AI-powered operating system for our providers, where clinical copilots deliver real-time insights and autonomous agents handle the administrative grind. We are growing quickly and have partnered with Medicare to offer affordable and exceptional psychotherapy to hundreds of patients. We are honored to have the support and backing of world-class founders from Ramp, Nourish, Headway, Charlie Health, and Spring Health.

Why Work With Us

We are one of the fastest-moving teams in health tech: every person punches above their weight and owns meaningful work far beyond their title. We’re lucky to be backed by a tier 1 VC (unannounced) and the world-class founders of Ramp, Headway, Charlie Health, and Nourish.
