This role is first shift, Monday - Friday 9:00a - 5:00p.

Position Details
The Blended Case Management is designed to serve adults living in the community who have serious and persist mental illness. Using an individualized approach to service delivery, the Blended Case Management program strive to promote recovery, independence, and community integration. The Blended Case Manager is responsible for developing, coordinating, and implementing services for adult participants as identified in the goal-oriented service plan. The role includes providing resource coordination, recovery services, and symptom management using a strength-based model of case management to 25-30 Adults living with mental illness. The Blended Case Manager will also provide information and support to families and significant others of the individuals served as well as complete required paperwork, correspondence and chart documentation.

Preferred QualificationsThe ideal candidate will possess one the following qualifications:
- A high school diploma plus a minimum of 12 college credit hours in Psychology, Sociology, Social Welfare, Gerontology, or other social science required and two (2) years of behavioral health direct care experience required; or
- A Bachelor’s degree with major course work in sociology, social work, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, healthcare administration, counseling, or education;
The candidate will have all of the following:
- Must possess a valid driver's license, insurance and have a clean driving record.
- Own a reliable vehicle.
Skills Required
- High school diploma plus at least 12 college credit hours in psychology, sociology, social welfare, gerontology, or another social science, and two years of behavioral health direct care experience
- Alternatively, a bachelor's degree with major coursework in sociology, social work, social welfare, psychology, gerontology, anthropology, related social sciences, criminal justice, theology, nursing, healthcare administration, counseling, or education
