Community Health Alliance is seeking a Bilingual Case Manager/QBHS who is fluent in both Spanish and English to provide culturally responsive, community-based behavioral health services and support to Spanish-speaking children, adolescents, adults, and families.  This role is community based and includes home visits, service coordination and collaboration with providers, families and other supports.

What You'll Do

  • Coordinates assessments, treatment planning or crisis intervention services.
  • Facilitates healthcare, behavioral health and other services and supports as part of care coordination.
  • Provides training and facilitates linkages for the use of community resources.
  • Monitors symptoms and service delivery to ensure client needs are met.
  • Obtains or assist clients in obtaining necessary services for basic necessities and services.
  • Assists clients in accessing self-help support groups and increasing social support networks.
  • Provides appropriate interventions to aid clients in the restoration of daily living skills.
  • Coordinates and assist in crisis management and stabilization.
  • Collaborates and communicates effectively with treatment team for the coordination of care.
Qualifications

Required Qualifications

  • High School diploma plus 3 years of direct experience. Associates or Bachelor’s degree in a social sciences field or related field is preferred. 
  • Spanish/English bilingual.
  • An understanding of mental illness, psychiatric symptoms, and impact on functioning and behavior.
  • An understanding of how to therapeutically engage a person with mental illness.
  • Valid driver's license, car insurance, and insurable driving record required.
  • Willingness to travel throughout the assigned service area.
  • Must have reliable transportation.
  • An understanding of the community mental health system.
