POSITION: Resident Care Coordinator

REPORTS TO: Health and Wellness Director

FLSA: Exempt

POSITION SUMMARY

The Resident Care Coordinator supervises and evaluates the care of residents in a memory care and assisted living facility. Schedules and trains care staff, and ensures the clinical services are delivered in accordance with regulations and professional standards.

ESSENTIAL JOB FUNCTIONS:

• Supports the mission, vision, and goals of the organization, upholding and promoting company culture and vision.

• Coordinating patient care plans by liaising with all necessary care providers and medical professions to ensure patient needs are met.

• Coordinate follow-up appointments and refer patients to specialists as needed.

• Looks for ways to improve the quality of patient care provided by the facility.

• Works to create efficient systems and workflows within the clinic.

• Attend departmental meetings and stay up to date on all business initiatives related to the clinic.

• Meet with patients and patient families to ensure their needs are being met and resolve any issues or complaints that arise.

• Attends training courses at community and maintains up to date monthly, quarterly, annual training through designated Learning Management System (LMS).

• Other duties as assigned, and which relate to the success of the community and the care, comfort, and happiness of our residents.

Requirements

EXPERIENCE, QUALIFICATIONS & SKILLS:

• Previous experience in assisted living.

• A high school diploma or equivalent.

• Meets all mandatory health and training requirements.

• Maintains CPR and First aid certifications.

• Demonstrates basic computer skills.

• Able to work a flexible schedule, including weekends, holidays, and overtime, to meet the requirements of the position.

• Be able to relate positively and communicate appropriately with residents, family,

community members, volunteers, and other employees.

• Ability to work independently.

• Must have strong understanding of the English language sufficient to read and write and

interpreting medical and administrative information.

PHYSICAL DEMANDS:

The following physical demands are representative of those that must be met to successfully perform the essential functions of this position:

• Walk/Stand – must be able to continuously walk and stand.

• Environment Condition – must be able to perform work both inside and outside.

• Sit – sit infrequently.

• Lift - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds.

• Carry - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds.

• Push - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds.

• Climb – must be able to climb stairs on a frequent basis, with the opportunity to utilize the elevator as an alternate resource if available.

• Bend – must be able to bend at the waist, knees, hips, and spine on a frequent basis; may require frequently manipulating weights of 25 pounds or more.

• Squat – must be able to squat on a frequent basis to reach below the knee, may require lifting of 50 pounds or less.

• Twist or rotate – must be able to twist or rotate occasionally at the trunk, hips, knees, and neck, manipulating weights of 25 pounds or more.

Skills Required

  • Previous experience in assisted living.
  • MedTech certification or LPN license.
  • High school diploma or equivalent.
  • Meets all mandatory health and training requirements.
  • Current CPR and First Aid certifications.
  • Basic computer skills and ability to use a Learning Management System (LMS).
  • Ability to work a flexible schedule, including weekends, holidays, and overtime.
  • Strong verbal and written English communication skills for interpreting medical and administrative information.
  • Ability to relate positively with residents, families, community members, volunteers, and staff.
  • Ability to work independently and as part of a team.
  • Maintain up-to-date monthly, quarterly, and annual training.
