Essential Duties and Responsibilities:
This position falls within the Monitoring Department of Uprise Health.
This department provides services to professional licensing boards, governmental agencies, private employers and families. Services are provided to employees / licensees / dependents who have been identified with a substance use and/or mental health disorder.
Services include evaluations, monitoring compliance with treatment requirements, monitoring/approving medications, partnering with workplace supervisor to ensure workplace safety, periodic check-ins with participant by Uprise Heath licensed staff, and toxicology services. Toxicology includes scheduling tests on appropriate panels, maintaining a notification system to alert participant if testing is required each day, verifying that testing was completed as required and monitoring test results. Contract-holders and other stakeholders receive reports and frequent communications regarding compliance and status.
- Act as an Agreement Monitor I for assigned participants. Being an “Agreement Monitor I” means providing comprehensive case management duties, overseeing the compliance of clients (“participants”) to their treatment plans (“monitoring agreements”). It means making independent judgement and being responsible for the management of the participant’s case throughout program participation, beginning with completion of an initial intake until successful discharge or termination from the program.
- Prepare monitoring agreements, releases, letters and other documents for assigned participants.
- Monitor the status of all requirements of the monitoring agreement for assigned participants and update the monitoring agreement annually or more frequently as needed.
- Complete intakes with newly assigned participants, via phone, video conference or in-person.
- Conduct check-ins with participants (via phone or email)
- Monitor participant’s workplace performance through regular contact with employer and receipt of monthly reports.
- Collaborate with participants’ treatment providers to ensure receipt of regular documentation
- Determine toxicology testing schedules for participants and monitor receipt of all toxicology results, taking immediate action in the case of positive tests.
- Respond to incidents of non-compliance with the monitoring agreement quickly, which may include writing reports and informing licensing Boards, employers, and treatment providers.
- Assist in determining and implementing appropriate action plans for any high-risk issues.
- Provide updates and reports to the referring entity (e.g. licensing board or employer).
- Oversee all tasks related to the provision of services for assigned participants to ensure they are completed within designated time frames.
Essential Education and Experience:
- High school diploma and minimum 5 years experience in a behavioral health discipline are required
- Case management background is essential.
- Experience working with individuals with substance use and mental health conditions is preferred.
- Robust critical thinking skills
- Strong attention to detail
- Excellent written and verbal communication skills
- Comfortable and experienced with working in a web-based software system, as well as with Microsoft Office Products
- Ability to work well in a team environment
- Ability to excel in a fast-paced environment
