Job Summary:

ACT Team Supervisor is a professional specialist on the team whom works closely with the Director of ACT Team. The ACT Supervisor assumes the leadership functions, responsibilities and clinical oversight in collaboration with the Director of ACT team and other assigned duties during their absence. Supervisor will ensure the team meets its performance outcomes as defined by the NYS Office of Mental Health licensing standards, and Joint Commission accreditation standards. Works in collaboration with the Director of ACT Team, and all clinical ACT staff to ensure the team reaches monthly targets while promoting patient centered care and positive patient outcomes. Supervisor will work with the team to ensure that the Fidelity of the ACT model is maintained.

Duties/Responsibilities:

  • Oversees the clinical and administrative operations of the ACT Team and reports on status of the team’s performance to the Director of ACT Team
  • Assists with leading the daily organizational meeting and weekly clinical team meeting.
  • Coordinates the intake of new participants in collaboration with interdisciplinary team.
  • Direct team members to complete daily service delivery to program recipients to ensure consumers receive services based on current needs and risks.
  • Provide person-centered service coordination to the team participants.
  • Attain monthly and annual productivity requirements.
  • Collaborate with the participant in the formulation and ongoing updates of treatment plans and other required assessments.
  • Engage in assertive outreach as relevant.
  • Support the program participants in ensuring successful living in their homes (provide skills teaching, ADL coaching, assistance with cleaning and laundry, etc.)
  • Complete service docs and all relevant documentation in accordance with agency policies.
  • Maintain written and computerized records, compile reports and complete other program documentation (e.g., service docs, incident reports, on-call logs, letters, etc.).
  • Organize and participate in social activities with participants in the community.
  • Coordinate and monitor referrals to community services and advocate for participant’s active involvement in the process.
  • Participate in on-call rotation.
  • Provide supervision to staff according to the agency policies.
  • Provide 24/7 on call backup for Director of ACT Team as needed.
  • Assume regulatory and fiscal reporting requirements in Director of ACT Team’s absence.
  • Participate in meetings as required (Housing meeting, Clinical Meeting, the Vacancy meeting, etc.)
  • Support the Director of ACT Team and the team members in carrying out daily tasks.
  • Perform responsibilities to support team members as assigned by the Director of ACT Team.
  • Complete other duties as assigned.
  • Promote and monitor the team’s performance to ensure the team achieves positive outcomes in terms of reducing homelessness, reducing participant hospitalization and emergency room visits, reducing drug use, reducing participant use of dangerous behaviors, and reducing criminal recidivism.

Required Skills/Abilities (Unit Specific):

  • Demonstrated leadership skills and ability to interface effectively with employees, patients, and community.
  • Ability to deal under stressful situations often dealing with patients, families, and subject to irregular hours and on-call responsibilities.
  • Knowledge of community resources
  • Excellent oral and written communication skills and computer literacy
  • Experience in a management or supervisory capacity preferred
  • Must be knowledgeable of the ACT model with a willingness to promote and respect the philosophy

Education and Experience:

• Preferred Master’s degree in psychology, social work, or related field, or Bachelor’s degree and minimum 5 years of experience working directly on an ACT Team.

• Experience working in community mental health settings that provides services to recipients in the community.

Working Conditions:

  • Requires attendance of seminars, workshops, conferences when appropriate.
  • Requires attendance of staff and team meetings.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Ability to perform prolonged or extensive standing, walking, and/or traversing.
  • Must be able to lift up to 15lbs at times.
  • Comply with DOH requirements for annual health assessments and required immunizations.

Pay Range: $27.67-$38.00/hr

Employee salary is based on commensurate experience and other qualifications.
