Job Summary

The ACA Coordinator is responsible for ensuring organizational compliance with the Affordable Care Act (ACA) while also supporting patient care transitions. This role works closely with the Discharge Planner to coordinate aftercare services, assist patients with insurance enrollment, and connect individuals to community-based resources. The ACA Coordinator plays a critical role in helping patients secure coverage, access care, and successfully transition back into the community.

Key Responsibilities

ACA Compliance & Administrative Responsibilities

  • Monitor and track employee and patient eligibility related to ACA compliance requirements
  • Maintain accurate records in accordance with federal and state ACA regulations
  • Prepare and support ACA reporting requirements as applicable
  • Partner with HR, Benefits, Payroll, and external vendors to ensure data accuracy
  • Stay current on ACA regulations and communicate updates to leadership

Patient Care Coordination & Community Support

  • Collaborate with the Discharge Planner to schedule and coordinate aftercare medical and behavioral health appointments
  • Assist patients with enrolling in Medicaid and other applicable health coverage programs
  • Support patients in enrolling in community-based programs and social service resources
  • Serve as a liaison between patients, healthcare providers, and community agencies
  • Provide guidance and support to patients to help them stabilize, access care, and successfully reintegrate into the community
  • Document all coordination and enrollment activities in accordance with organizational policies

Work Environment

  • Clinical or healthcare setting (on-site)
  • May involve direct patient interaction and coordination with external agencies

Reporting Structure

  • Reports to: Director of Social Services, Case Management Supervisor, or Human Resources Leadership
Qualifications

Qualifications

  • Required: Active LCSW (Licensed Clinical Social Worker) or CSW (Certified Social Worker) license
  • Bachelor’s or master’s degree in social work, Human Services, or related field
  • Experience with Medicaid enrollment, community resource coordination, and discharge planning
  • Knowledge of Affordable Care Act regulations and healthcare coverage programs preferred
  • Strong interpersonal, organizational, and documentation skills
  • Ability to work collaboratively with interdisciplinary healthcare teams
  • Commitment to patient advocacy and continuity of care

Preferred Qualifications

  • Experience in healthcare, corrections, behavioral health, or community-based settings
  • Familiarity with social determinants of health and case management practices
  • Experience working with underserved or high-risk populations

Skills Required
