For most older adults, healthcare doesn’t fail in one place—it fails across many. Mental health, physical health, medications, and day-to-day realities all interact, but the system treats them separately. The result is confusion, missed care, and worsening outcomes.
Sailor Health is building a new model of care for older adults that brings the mind and the body together. We combine behavioral health, medical insight, and hands-on care navigation to help patients and families understand what’s going on and what to do next. By taking a more holistic and preventative approach, we aim to catch issues earlier, reduce avoidable crises, and keep patients in their homes instead of the hospital.
⭐ About the RoleYou are the clinical gateway for patients navigating chronic conditions and complex care needs. Through telehealth visits, you assess the full medical picture and partner closely with healthcare advocates to define and oversee care plans that patients can actually follow.
While therapists lead the work on the mind, you lead the clinical perspective on the body—bringing medical clarity to situations that are often fragmented or poorly coordinated.
You will be one of the first physicians in this role at Sailor, and you’ll play a key part in shaping how it evolves. We’re looking for someone who is excited by growth, comfortable with change, and motivated by the opportunity to help build something from the ground up while delivering excellent care.
💻 Your Key ResponsibilitiesConduct comprehensive telehealth assessments for older adults with chronic and complex conditions
Identify clinical risks and gaps in care across diagnoses, medications, and recent utilization
Oversee patient care plans in partnership with healthcare advocates, addressing both medical needs and social determinants of health
Collaborate with therapists and care teams to ensure physical health is fully integrated into ongoing care
MD or DO, licensed in at least 45 states including Florida, Maryland, Pennsylvania, and Texas
Board certification in Internal Medicine, Family Medicine, or related specialty
At least 5 hours of availability a week
Experience caring for patients with chronic and complex conditions, especially older adults
Strong clinical judgment and ability to synthesize incomplete information into clear plans
Comfortable using technology and excited about leveraging AI to improve care delivery
Passionate about mental health and holistic care
Skills Required
