Teams at Westmoreland Casemanagement and Supports, Inc.

Provide targeted case management for children, transition-age, and adult clients: assess needs, develop and implement service plans, coordinate resources, monitor services, perform crisis intervention, document care, and engage in community outreach and interdisciplinary teamwork.

Provide school-based Student Assistance Program services: screen and assess students for behavioral health and substance use issues; consult with school teams and families; refer and follow students to services; co-facilitate psychoeducational groups; deliver trainings; respond to mental health crises and assist with involuntary commitment documentation.

Provide targeted case management across child, transitional age, and adult populations. Assess client needs, develop and implement service plans, coordinate and monitor services, perform crisis intervention, advocate for clients, maintain documentation, and participate in community outreach and interdisciplinary teams. Work includes face-to-face contacts, variable schedules, on-call rotation, and frequent community-based visits.
