Conduct outreach in jails, courts, hospitals, and the community; develop care plans, coordinate transportation, attend court, connect clients to housing, benefits, and services, document clinical interactions, and collaborate with multidisciplinary teams to support recovery and legal diversion outcomes.

Provides vocational rehabilitation, employment assessments, job coaching, placement, and skill-building for people with serious and persistent mental illness. Develops community employment opportunities, builds employer and agency partnerships, coordinates with clinical teams, verifies program eligibility, supports workplace success, and completes program reporting. The role works in the community approximately 60% of the time and may require occasional travel between clinical sites.

Develops and expands organizational safety programs for outpatient and residential behavioral health services. Conducts site audits, safety meetings, emergency drills, training, incident investigations, and compliance activities. Identifies safety trends and risks, supports corrective plans, manages surveillance footage, responds to safety inquiries, and coordinates with law enforcement and emergency responders. The role also participates in disaster response and supports risk management projects across WellPower locations.
