Provide case management and service linkages for residents in a supported housing program: assess apartments and resident needs, coordinate community and treatment services, assist with housing/subsidies/benefits, supervise staff, document incidents, conduct urine testing, respond to emergencies (on-call 24/7), and complete required trainings (AMAP, CPR, First Aid).

Provide assessments, individual and group counseling, case management and linkage to services. Develop treatment plans with ACT team, facilitate groups, ensure documentation and OMH compliance, provide crisis intervention, coordinate entitlements and transportation, enter data into VelloHealth, and operate agency/own vehicle for home visits.
