Provides skilled nursing support in a senior rehabilitation and skilled nursing facility. Responsibilities include MDS assessments, restorative programming, admission orders, head-to-toe admission assessments, care plan meetings, rounds, audits, and interdisciplinary team participation. The role works Monday through Friday, 9:00 a.m. to 4:00 p.m., with an on-call rotation every five weeks.

Prepare weekly nursing schedules, maintain staffing coverage, coordinate with caregivers, and assist the nursing department with administrative projects. The role requires strong organization, multitasking, time management, communication, dependability, integrity, and a caring, professional attitude while supporting quality resident care.

Coordinates the MDS assessment process, collaborates with interdisciplinary teams to develop resident care plans, mentors staff, and provides nursing leadership in a long-term care facility. The role requires an RN with nursing leadership experience, current CPR certification, strong communication skills, and a commitment to resident-centered care.
