The MDS Nurse LPN coordinates resident assessments, gathers clinical information, determines assessment timelines, completes accurate MDS coding, and ensures compliance with state and federal regulations. The role collaborates with nursing, therapy, social services, residents, families, and other interdisciplinary team members to support individualized care and maintain accurate assessment schedules.

Provides social work services to nursing and rehabilitation center residents, including assessments, care planning, advocacy, emotional support, family meetings, discharge planning, and connections to community resources. The director maintains documentation, supports admissions and transitions, educates residents and families, and collaborates with interdisciplinary healthcare teams while managing department supplies and budget.
