Provides peer-based support and navigation to individuals with medical, behavioral health, mental health, substance use, HIV, and chronic illness needs. Conducts outreach, appointment reminders, care re-engagement, testing and counseling, peer groups, and appointment accompaniment. Maintains timely, accurate documentation in AIRS and eClinicalWorks, performs data entry for PrEP and RAP programs, resolves discrepancies, supports audits and quality improvement, and follows confidentiality and reporting requirements.

Provide peer support and outreach to clients with HIV, mental health, medical, or substance use histories. Conduct in-person outreach, follow-up calls/letters, accompany clients to appointments, perform home/agency visits, facilitate support groups, represent the agency at events, maintain HIPAA compliance, and participate in trainings and special projects.

Provides individual and group counseling, assessments, crisis intervention, care planning, case management, and community resource coordination for patients with Opioid Use Disorder. Maintains a caseload of at least 30 patients, completes clinical documentation, participates in case conferences and quality improvement activities, and supports evidence-based substance use and mental health services.

Manage Medicaid/HRA intake and re‑applications, collect and maintain client documentation and filing systems, verify eligibility, request Fair Hearing/SSI appointments, process Managed Care dis-enrollments and EFSP vouchers, produce weekly reports, liaise with HRA, support re-billing and self-pay collection, and perform related administrative duties across programs.

Provide 100% clinical services to adolescents and families in a school/clinic setting: perform DSM diagnostic assessments, develop treatment plans, implement evidence-based interventions, facilitate groups, participate in interdisciplinary team meetings, and consult with psychiatry and mental health specialists.

Provide direct nursing care under the Nurse Manager: administer medications, observe for intoxication and side effects, document medical records, assist clients with care plans, perform health teaching/counseling, maintain medication accountability, participate in trainings and committees, and perform duties within scope of practice.

Lead day-to-day compliance operations across multiple facilities, supervise compliance staff, manage investigations and incident tracking, ensure regulatory adherence (HIPAA, 42 CFR Part 2, OMIG), develop training, prepare regulatory responses and reports, and coordinate cross-departmental compliance activities in support of the CCO.

Manage patient scheduling, insurance verification, check-ins, document scanning, and incoming calls for Health Services. Maintain patient records in ECW, collect co-pays, prepare deposits, coordinate supplies, liaise with staff and outside agencies, and support department operations while ensuring confidentiality and attendance at required meetings.
