The Case Manager provides comprehensive case management services to clients, collaborates with healthcare professionals, and advocates for clients' needs, ensuring quality care and access to resources.

Provide comprehensive case management for clients receiving HIV care at non-Vivent clinics: assess needs, create care plans, coordinate medical and social services, deliver crisis intervention, document activities, advocate for clients, and collaborate with interdisciplinary teams.

Supervise and mentor case managers to deliver high-quality case management services. Oversee development and execution of care plans, ensure documentation and compliance, monitor performance, provide training, analyze data for improvements, coordinate with stakeholders, and drive continuous quality improvement.

The Psychotherapist is responsible for providing patient-centered psychotherapy services, including intake, assessment, treatment planning, and participation in administrative tasks supporting the clinic's operations.

The Case Manager coordinates comprehensive care for clients, assessing their medical and psychosocial needs and providing advocacy and support within the healthcare system to enhance their well-being.

Provide comprehensive case management within a clinic: assess medical and psychosocial needs, develop individualized plans, coordinate care and resources, perform eligibility assessments, provide crisis intervention, document activities, collaborate with interdisciplinary teams, and support patient education and advocacy.

Provide evidence-based individual, couple, and family psychotherapy for patients in an integrated HIV care clinic. Conduct intake, assessment, treatment planning, therapy, documentation, referrals, care coordination, and follow-up. Participate in supervision, staff meetings, trainings, administrative and billing tasks, on-call coverage, and community/provider collaboration. Support culturally sensitive, trauma-informed care and may obtain additional certifications as needed.
