Provides patient-centered outpatient nursing care for patients living with or at risk for HIV. Conducts assessments, administers medications and injections, performs clinical procedures, documents in EHRs, educates patients, triages needs, coordinates referrals, collaborates with interdisciplinary teams, supports clinic operations, contributes to quality improvement, and participates in research and continuing education.

Leads enterprise-wide people strategy and HR operations, advising the CEO and executive team on workforce planning, talent, culture, organizational design, succession, compensation, benefits, employee relations, compliance, DEIB, and leadership development. Oversees multi-state recruitment, HRIS/HCM modernization, people analytics, and service delivery while partnering with the board and executive leadership to strengthen organizational effectiveness and workforce readiness.

Supports organization-wide healthcare quality improvement by compiling and analyzing data, monitoring workplan goals, identifying care gaps, coordinating performance reporting, and supporting quality committees. The role assists with Press Ganey administration, HRSA and Ryan White reporting, audits, PDSA cycles, patient-centered medical home recognition, consumer advisory activities, staff training, and coordination among clinical, operational, social service, and prevention teams.

Provide client-centered enrollment and benefits assistance for people with HIV/AIDS, coordinating insurance outreach, ACA/Marketplace enrollment, staff training, documentation in EPIC/Provide Enterprise, referrals to case management, and monitoring policy changes to ensure access to care.

Provides comprehensive case management for clinic patients, including medical and psychosocial assessments, individualized care plans, healthcare coordination, eligibility verification, community resource referrals, advocacy, crisis intervention, health education, and documentation. Collaborates with interdisciplinary healthcare teams to support holistic outcomes for people living with HIV and may assist other clinic service lines as needed.

Provide individual, couples, and family psychotherapy within an interdisciplinary behavioral health clinic. Conduct intakes, assessments, treatment planning, therapy, discharge and follow-up; treat co-occurring mental health and substance use disorders; participate in administrative, billing, and program duties; coordinate care with community providers; attend supervision and trainings; may provide on-call/after-hours services and pursue additional required certifications or training.

Provide comprehensive case management for clients receiving HIV care at non-Vivent clinics: assess needs, create care plans, coordinate medical and social services, deliver crisis intervention, document activities, advocate for clients, and collaborate with interdisciplinary teams.

Provide clinic-based case management for patients, conducting assessments, developing individualized care plans, coordinating medical and social services, performing eligibility documentation for grant-funded services, crisis intervention, advocacy, documentation, health education, and collaborating with interdisciplinary teams to improve health outcomes.
