Provide comprehensive case management for clients receiving HIV care at non-Vivent clinics: assess needs, create care plans, coordinate medical and social services, deliver crisis intervention, document activities, advocate for clients, and collaborate with interdisciplinary teams.

Supervise and mentor case managers to deliver high-quality case management services. Oversee development and execution of care plans, ensure documentation and compliance, monitor performance, provide training, analyze data for improvements, coordinate with stakeholders, and drive continuous quality improvement.

Provide clinic-based case management for patients, conducting assessments, developing individualized care plans, coordinating medical and social services, performing eligibility documentation for grant-funded services, crisis intervention, advocacy, documentation, health education, and collaborating with interdisciplinary teams to improve health outcomes.

Provide comprehensive case management for clients living with or at risk for HIV: assess medical and psychosocial needs, develop care plans, coordinate services and community resources, perform eligibility assessments, provide crisis intervention, document activities, advocate for clients, and collaborate with interdisciplinary teams to improve health outcomes.

Provide client-centered enrollment and benefits assistance for people with HIV/AIDS, coordinating insurance outreach, ACA/Marketplace enrollment, staff training, documentation in EPIC/Provide Enterprise, referrals to case management, and monitoring policy changes to ensure access to care.

Provide evidence-based individual, couple, and family psychotherapy for patients in an integrated HIV care clinic. Conduct intake, assessment, treatment planning, therapy, documentation, referrals, care coordination, and follow-up. Participate in supervision, staff meetings, trainings, administrative and billing tasks, on-call coverage, and community/provider collaboration. Support culturally sensitive, trauma-informed care and may obtain additional certifications as needed.
