Provide comprehensive case management for clients living with or at risk for HIV: assess medical and psychosocial needs, develop individualized plans, coordinate medical and social services, perform eligibility assessments, offer crisis intervention and advocacy, document activities, and collaborate with interdisciplinary teams to improve client outcomes.

Provide client-centered enrollment and benefits assistance for people with HIV/AIDS, coordinating insurance outreach, ACA/Marketplace enrollment, staff training, documentation in EPIC/Provide Enterprise, referrals to case management, and monitoring policy changes to ensure access to care.
