Drives referral growth and admissions within an assigned territory through in-person relationship building, outreach, cold calling, networking, and healthcare partner engagement. Develops referral relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other providers. Tracks sales activity in CRM systems, meets performance targets, supports patient transitions with clinical and operational teams, and represents the organization at community events and educational engagements.

Provide administrative support for a home health/hospice branch: manage office functions, handle new-hire I-9s and orientation, process mail, answer calls, order supplies, and maintain electronic records.

Own month-end sales and executive reporting, build market share and referral analytics, maintain dashboards, track workforce and productivity metrics, validate data governance, and support budgeting and strategic reporting to drive growth for Home Health and Hospice.
