Maintains confidentiality and professional communication with patients, families, healthcare professionals, team members, and community agencies while supporting the agency’s mission and daily operations.

Drives referral growth and patient admissions within an assigned territory by building relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other healthcare providers. Responsibilities include field-based prospecting, cold calling, networking, referral partnership management, CRM activity tracking, performance-target achievement, community representation, and coordination with clinical and operational teams.

Drive referral growth and admissions within an assigned territory by building relationships with healthcare providers, generating new business through outreach and networking, meeting sales targets, managing referral pipelines in CRM systems, and coordinating with clinical and operational teams to support patient transitions.
