The Justice Reentry Coordinator supports individuals with serious mental illness transitioning from correctional facilities, ensuring care coordination, service referrals, and community integration.

Provide community outreach, education, and training on the Transitions to Community Living (TCL) program and RSVP process. Identify and resolve service barriers, coordinate care across providers and agencies, document interventions in consumer records, support DOJ reporting, and collaborate with stakeholders to facilitate transitions and diversion efforts.

Provide screenings, case management, and care coordination to divert individuals at risk of entering Adult Care Homes. Complete state-required documentation, develop community integration plans, educate members about options, coordinate services across providers, and build collaborative relationships with stakeholders. Work is field-based with travel and use of state systems.

Provide community-based transition planning and coordination for behavioral health members leaving institutional settings. Manage caseloads, develop person-centered plans, arrange housing and services, liaise with providers and stakeholders, document transitions, and perform follow-up to ensure continuity of care.

Provide transition planning and coordination for individuals discharging from institutional settings to community-based services. Manage caseloads, develop person-centered transition plans, coordinate community resources and providers, document interventions and outcomes, and support follow-along and move-in logistics. Requires collaboration with clinical teams, institutions, and stakeholders to ensure safe, timely transitions.

Perform community-based screenings and case management to divert individuals from Adult Care Homes, coordinate care and treatment planning, document in state systems, educate members, and collaborate with providers and stakeholders to support community integration.

Provide in-reach, education, and peer support to adults with serious mental illness about community-based services and supported housing. Conduct face-to-face visits, assess housing interest, accompany site visits, document outcomes, coordinate with Vaya staff, and participate in trainings and state-level initiatives.
