Lead and coordinate implementation of ROOTS Hub Initiative 1 across NC region 3, engaging healthcare, behavioral health, and social service partners; manage operations, stakeholder and community engagement, reporting, compliance, and performance monitoring to improve rural healthcare access.

Lead implementation of Initiative 2 across NC Region 3, coordinating healthcare, behavioral health, and community partners; manage project plans, monitor performance, support reporting, ensure compliance, and engage communities to scale prevention, chronic disease management, maternal and nutritional health programs.

Lead implementation and operational oversight of Initiative 5 across NC Region 3 to advance value-based payment readiness, fiscal sustainability, and rural primary care transformation. Coordinate stakeholders, provide technical assistance, monitor performance and compliance, support reporting, and foster community engagement to establish ROOTS Hubs connecting medical, behavioral health, and social services.

Manage and coordinate QAPI activities for the ROOTS Hub, develop and implement QA processes, design and run performance improvement projects, analyze quality data, produce reports, support Hub compliance and reporting, and facilitate communication between Hub leadership and governance bodies.

Provide reporting and data support for the ROOTS Hub region, compile and submit program reports, analyze and monitor performance measures, ensure compliance, and present findings and recommendations to internal and external stakeholders.

Manage planning, coordination, execution, and monitoring of ROOTS Hub projects. Develop project charters, schedules, risk logs, and governance materials; engage stakeholders, facilitate readiness and transitions, track metrics and budgets, and support adoption and continuous improvement across cross-functional teams.

Manage RHTP program finances including budgeting, accruals, reconciliations, accounts payable, payments to sub-grantees, and financial/compliance reporting. Ensure expenditures comply with federal, state, and CMS guidelines, support audits, monitor spending variances, and recommend process improvements while collaborating with stakeholders.

Provide clinical intervention and care coordination to individuals in permanent supported housing under the Transitions to Community Living program. Monitor housing stability, stratify risk, prevent separations, lead rehousing efforts, complete financial reassessments and documentation in EHR/TCLD, coordinate with providers and stakeholders, and comply with PHI/HIPAA requirements while maintaining a caseload and supporting program reporting.

Provide peer-based case management and reentry support to individuals with serious mental illness released from DAC. Coordinate with DAC, treatment providers, housing, benefits, and community stakeholders; ensure timely documentation, referrals, discharge planning, and person-centered care while conducting field visits and travel in the community.

The Justice Reentry Coordinator supports individuals with serious mental illness transitioning from correctional facilities, ensuring care coordination, service referrals, and community integration.
