Facilitate complex discharge planning and care coordination by assessing, planning, implementing, and evaluating safe discharge plans. Collaborate with physicians, RNs, and MSW case managers; maintain knowledge of community and post-acute resources; communicate with patients and families; provide weekend/holiday coverage, on-call duties, and travel between facilities as needed.
