Provide intensive post-release case management for unaccompanied children and sponsoring families: stabilize households, deliver trauma-informed clinical interventions, conduct in-home visits and screenings, coordinate community referrals, document per KYUMH/ORR standards, and provide crisis support including evenings, weekends, and frequent travel.

Serve as primary liaison for youth and families from admission to discharge. Complete admissions, assessments, and Individual Service Plans; coordinate services, documentation, and case meetings; screen and assist sponsors for discharge. Engage youth and families therapeutically, communicate with staff and community partners, and maintain on-call and flexible evening/weekend availability. Fluency in English and Spanish required.

Oversee PSA compliance and quality improvement for Caminos Shelter and HS/PRS programs, ensuring adherence to ORR, state, and agency standards; develop and deliver training; investigate PSA grievances; monitor data entry and compile regulatory and program outcome reports.

Provide person-centered Comprehensive Community Support Services (CCSS) including assessments, individualized service plans, skill-building in home/community settings, documentation per Medicaid/agency standards, staff supervision support, and interdisciplinary collaboration under clinical supervision.

Coordinate and oversee clinical services for the Caminos youth program, ensure compliance with documentation and trafficking screening, provide assessments, individual and group therapy, crisis intervention, clinical guidance to staff, staff training, and serve on on-call rotation while maintaining a clinician caseload.

Lead Clinician coordinates and oversees clinical services for youth in the KYUMH Caminos program, ensuring documentation and regulatory compliance, providing clinical guidance and training, participating in care team meetings, conducting mental health assessments, delivering individual and group therapy, crisis intervention, and supervising/mentoring assigned clinicians while serving on the on-call rotation.
