Supervise and support case management staff in transitional housing for formerly incarcerated clients. Oversee intake, clinical assessments, individualized treatment plans, documentation, crisis intervention, referrals to benefits and services, and funder reporting. Provide weekly supervision, training, performance management, and carry a small caseload when needed. Serve as on-call clinical support for emergencies and ensure program compliance with agency and funder standards.

Provide case management and counseling to formerly incarcerated, homeless, or at-risk residents. Screen, assess, develop treatment plans, facilitate individual and group sessions, link clients to community resources, maintain records and interagency liaisons, write court reports, and perform urinalysis under Program Director supervision.

Facilitate evidence-based workshops and complete individualized discharge/transition plans for incarcerated participants; coordinate with internal staff and community providers, enter program data, monitor confidentiality and safety, and support participants' successful reentry into community or alternative placements.

Support Senior Controller and Director of Accounting to maintain internal controls, manage month-end close, oversee payroll/accounts payable backups, assist with government grants/contracts and claims, coordinate audits and tax filings (Form 990/1099), manage cash, property rental portfolio, insurance and retirement plans, and supervise accounting staff and vendors.

Manage end-to-end recruitment and onboarding for ~150 annual hires. Partner with hiring managers, ensure compliant requisitions and background screenings, maintain recruitment data and dashboards, train managers on best practices, coordinate orientation, and improve candidate experience aligned with Fortune's mission.

Provide intake, assessment, counseling, case planning, crisis intervention, housing placement assistance, benefits application support, documentation, room inspections, referrals, and interdepartmental coordination for formerly incarcerated and homeless participants with behavioral health or HIV histories.

Deliver evidence-based workforce-readiness and cognitive behavioral workshops in correctional and community settings; assess participants, develop work readiness portfolios, document attendance and progress in case management systems, collaborate with staff and volunteers, assist with outreach, onboarding, mock interviews, and follow-up to support successful reentry.

Provide one-on-one and group mentoring for participants in a gun diversion program; coordinate pro-social activities, family engagement workshops, and community events; develop client-centered plans, make referrals, monitor attendance and compliance, participate in case conferences, and engage in ongoing mentor training and professional development.

Document services, perform crisis intervention, assist with benefits and housing paperwork, facilitate groups, conduct room inspections, collect urinary specimens, and coordinate with internal and external providers to support participant reentry goals.

Provide intakes and manage a caseload of justice-involved participants, deliver individual sessions and group facilitation, develop treatment plans, document progress for court reports, coordinate referrals and toxicology tracking, perform data entry, engage in outreach and mentoring using lived experience, and collaborate across departments to support participant compliance and reentry goals.

Provide participant-centered screenings at Bronx Criminal Court to identify women needing mental health, substance use, or trafficking-related interventions; make real-time referrals, maintain referral tracking and GPRA data, liaise with court staff and treatment providers, and support program data reporting and occasional on-call coverage.

Perform data extraction, aggregation, and analysis for Behavioral Health programs (including DOC initiative). Run reports from CaseWorthy and EHR, monitor data quality, generate KPI and mid-month forecasting reports, coordinate with EQI and IT on dashboards, support billing backup, train staff, and provide data-driven recommendations to program leadership.

Supervise and manage the Health Home Care Management unit, ensuring regulatory compliance, staff supervision, client engagement, care coordination, data integrity, billing accuracy, partnerships and service transitions for individuals with complex behavioral health needs.

Facilitate Abusive Partner Intervention groups, provide individual case management and treatment planning, conduct assessments and toxicology screenings, maintain accurate documentation and court reports, supervise and mentor APIP staff, coordinate referrals, attend case conferences, and support program integrity and data entry.

Provide clinical supervision and crisis support to LMSW/LMHC staff, deliver individual and group therapy, conduct assessments and treatment planning, ensure timely documentation, participate in case conferences, onboard staff, provide occasional telehealth services, and coordinate care and referrals for clients with justice-involvement.

Supervise and support Transition Coordinators to ensure high-quality reentry services in jails and the community. Oversee program operations, data collection and reporting, administrative tasks, staff training, clinical oversight, crisis management, interagency liaison work, and coverage for the Director as needed.

Coordinate and manage the Queens County Reentry Taskforce, serve as primary liaison with DOCCS and DCJS, schedule and facilitate meetings and orientations, oversee case management staff, ensure submission of required reports and compliance with contractual milestones, coordinate service delivery and outreach, and notify stakeholders of barriers or critical incidents.
