Facilitate person-centered individual support plans, locate and coordinate providers, monitor services and documentation in HCSIS, schedule and run ISP team meetings, develop budgets and renewals, perform assessments and monitoring, promote health and safety, and maintain productivity standards.

The Addiction Clinician facilitates group and individual counseling sessions for substance abuse recovery, ensuring compliance with regulations and documentation standards. This temporary role has potential for permanent placement.
