Provides home- and community-based case management to homeless and unstably housed young adults. Conducts assessments, develops service plans, supports housing searches and tenancy, coordinates with treatment providers and community resources, provides transportation, builds money management and employment readiness skills, and completes documentation. The role serves approximately 8–12 clients and supports youth experiencing homelessness, substance use, behavioral health challenges, and family instability.

Provides strengths-based case management, supervision, job development, treatment support, and reentry guidance to justice-involved adult men in a residential work release program. The role helps clients transition from incarceration into the community through individualized services, rehabilitation, substance use and mental health education, and employment preparation. Requires compassionate, structured client support, professional boundaries, reliable attendance, a valid driver’s license, and either an associate degree or two years of related direct service and case management experience.

Supports program operations, quality assurance, reporting, training, and supervision for case managers serving adults experiencing homelessness, mental illness, or substance use disorders. Manages client census, admissions, case assignments, conferences, funder reviews, and administrative meetings. Provides leadership, on-call support, and coordination with outside agencies while helping clients maintain housing and improve daily functioning.

Conducts assessments, develops service plans, assists with housing, employment readiness, money management, transportation, and tenancy stabilization. Coordinates with treatment providers, landlords, and community resources while documenting services and managing an 8–12-person caseload. Supports youth and families affected by homelessness, substance use, and behavioral health challenges.

Provides comprehensive housing case management for individuals with severe and persistent mental illness, co-occurring disorders, homelessness histories, and housing instability. Responsibilities include assessments, case management meetings, home inspections, community resource referrals, individualized service planning, and coordination of housing stability goals. The role requires collaboration with clients and community providers, knowledge of local low-income housing resources, and experience serving homeless and SPMI populations.

Provides individual, family, and group clinical services and case management to pregnant and parenting women with substance use or co-occurring disorders. Collaborates with medical, employment, housing, and educational community partners; supports clients in the community; and provides consultation to the Women's REACH team. Moderate travel is required for client services, meetings, and training.

Provides clinical and administrative supervision for two residential mental health programs serving adults with chronic mental illness. Oversees program systems, service delivery, funder and constituent relationships, and supervises the Program Manager and Clinician. Requires a master’s degree with provisional or full licensure, at least five years of supervisory and related experience, knowledge of severe and persistent mental illness interventions, residential program management, DMHAS populations, and DPH regulations.
