Provides case management, supportive counseling, psychosocial activities, resident service coordination, treatment-team participation, interventions, and shift coverage for adults with serious and persistent mental illness in a transitional residential program.

Provides home- and community-based case management to homeless and unstably housed young adults ages 18–24. Conducts assessments, develops service plans, supports housing searches and tenancy, coordinates with providers and landlords, builds money management and employment readiness skills, provides transportation, and completes documentation for a caseload of 8–12 clients.

Provide strengths-based, client-centered case management, job development, supervision, and support to justice-involved men in a residential work-release program. Help clients maintain recovery, secure employment, and reintegrate into the community while coordinating with Community Justice staff and maintaining professional boundaries.

Oversee operations of a 24-hour residential work-release program for women on parole. Supervise staff, ensure contract and DOC compliance, provide on-call leadership, coordinate case management, job development, and community treatment linkages, and maintain relationships with state agencies and partners.

Provides staff support and direct client care within adult residential behavioral health treatment. Conducts intake assessments, develops and implements individual care plans, coordinates with treatment providers and community resources, and supports client recovery from mental illness, substance use, and homelessness under program supervision.
