Teams at The Connection

Provides case management, supportive counseling, psychosocial activities, resident service coordination, treatment-team participation, interventions, and shift coverage for adults with serious and persistent mental illness in a transitional residential program.

Provides home- and community-based case management to homeless and unstably housed young adults ages 18–24. Conducts assessments, develops service plans, supports housing searches and tenancy, coordinates with providers and landlords, builds money management and employment readiness skills, provides transportation, and completes documentation for a caseload of 8–12 clients.

Provide strengths-based, client-centered case management, supervision, and employment readiness support to justice-involved men transitioning from incarceration. Connect clients to health, education, and vocational services while maintaining structure, professional boundaries, and job development activities.
