Teams at The Connection

Provide home- and community-based case management to homeless young adults (18-24): intake assessments, housing search support, individual service plans, liaise with providers and landlords, transportation, money and employment readiness skills, documentation and data entry. Caseload ~8-12.

Provide strengths-based, client-centered case management, job development, supervision, and reentry support for justice-involved adults with psychiatric conditions. Coordinate with Community Justice staff to rehabilitate and guide clients from incarceration back into the community.

Provide strengths-based, client-centered case management, supervision, and employment readiness support to justice-involved men transitioning from incarceration. Connect clients to health, education, and vocational services while maintaining structure, professional boundaries, and job development activities.
