Teams at The Children's Place at Home Safe, Inc.

Conducts developmental screenings and assessments for mothers, babies, children, and families; interprets results; recommends services; coordinates referrals; and serves as a liaison among hospitals, families, supervisors, and community partners. The role includes home and community visits, data entry, documentation, and flexible evening or weekend appointments. Bilingual English/Spanish communication and local travel are required.

Provide in-home and community-based screening, assessment, referrals, and family education for children birth to five. Coordinate with entry agencies and community partners, document activities, and offer flexible scheduling including evenings/weekends to meet family needs.

Conduct eligibility screenings and assessments for new mothers and newborns, collaborate with hospital staff and registrars, identify client needs and connect families to program and community resources, maintain documentation and program data, promote Healthy Beginnings, attend trainings and staff meetings, and provide services at assigned hospitals with flexible scheduling.
