Provide in-home and community-based screening, assessment, referrals, and family education for children birth to five. Coordinate with entry agencies and community partners, document activities, and offer flexible scheduling including evenings/weekends to meet family needs.

Conduct eligibility screenings and assessments for new mothers and newborns, collaborate with hospital staff and registrars, identify client needs and connect families to program and community resources, maintain documentation and program data, promote Healthy Beginnings, attend trainings and staff meetings, and provide services at assigned hospitals with flexible scheduling.

Coordinate ELC referrals and family communications, schedule appointments, maintain HBDS records, support Quality Assurance Manager, prepare reports and materials, and monitor cases from intake to closure ensuring compliance and timely follow-up.

Coordinate Early Learning Coalition referrals and office services, schedule assessments and appointments, act as liaison with partners, prepare correspondence and reports, process mail, provide administrative and front-desk/data support, and ensure compliance with program requirements.

Provide in-home and community developmental screenings, parenting education, safe sleep and home-safety guidance, referrals to early intervention and healthcare, case coordination, documentation, and outreach for families with children 0–2 in Palm Beach County.
