The Occupational Therapist Assistant provides direct rehab care to patients under the supervision of an Occupational Therapist, following treatment plans, monitoring progress, and maintaining communication with healthcare team members.

Serve on the patient care team in a skilled nursing facility to plan, develop, and coordinate patient care. Provide discharge planning, family and community agency coordination, admissions support, documentation, and care plan input to ensure quality, compliant transitions.

The Unit Manager oversees nursing staff, manages admissions, audits documentation, reviews patient orders and medications, and ensures effective communication among staff.

Assist Physical Therapists in providing rehabilitative therapy to patients, ensuring compliance with regulations, documenting progress, and adjusting treatments as necessary.
