Supports person-centered planning and case management for individuals with intellectual and developmental disabilities. Responsibilities include coordinating PCP meetings, developing goals and service plans, monitoring documentation and authorizations, initiating funding requests, overseeing behavioral and health supports, maintaining compliance records, training staff, coordinating with external providers, monitoring benefits, and conducting regular contact and community visits.

Supervise and support day habilitation services for people with intellectual disabilities: schedule staff, review timesheets, ensure documentation matches billing, coordinate transportation, co-lead meetings, assist with person-centered planning, train and recruit staff, monitor medication delivery, and promote community-based skill-building and socialization.
