Provide nursing case management for assigned home health patients: develop and implement individualized care plans, perform initial and regular evaluations, document OASIS/PAR and clinical notes, coordinate with physicians, families and ancillary providers, administer medications, supervise ancillary staff, participate in quality reviews, take on-call shifts, and ensure regulatory compliance and timely documentation.

The Director of Nursing oversees patient care services and clinical staff, ensuring compliance with regulations while maintaining quality care and management of daily operations.

The Home Health Aide provides personal and skilled care to medically fragile children at home, following a plan of care under the supervision of a registered nurse.
