Teams at Team Select

Provides home health nursing case management for assigned patients, including developing and evaluating care plans, conducting assessments, administering medications, coordinating multidisciplinary services, educating patients and families, documenting care, and communicating with physicians. Supervises ancillary staff, participates in quality assurance activities, handles on-call triage, and ensures compliance with clinical, regulatory, and agency requirements.

Oversee an assigned home healthcare caseload and census by managing staffing, recruiting, scheduling, coverage gaps, and after-hours needs. Maintain communication with families and field staff, coordinate with nursing and case management teams, support referrals and intake, verify payroll, address insurance eligibility and authorizations, and help achieve branch financial and compliance goals.
