Provide case management and housing transition support to individuals and families experiencing homelessness. Conduct home visits, manage caseloads, complete housing applications/recertifications, enter case notes into HMIS/CHAMP, coordinate community referrals, develop individualized service plans, and support independent living and housing retention.

Provide street-based outreach, navigation, and supportive services to individuals experiencing homelessness; link clients to housing, health, and benefits; maintain HMIS records; coordinate with community partners and law enforcement; transport clients as needed; participate in meetings and trainings.

Provide intensive field-based case management for chronically homeless clients with severe mental health disorders. Manage caseloads, engage clients, link to housing and community resources, assist with subsidized housing applications, coordinate treatment plans with interdisciplinary FSP team, perform crisis assessment/intervention, maintain required documentation, and collaborate with partner agencies to support housing stability and independence.

Manage day-to-day benefits, leaves, workers' compensation, and core HRIS operations. Support HRIS implementations, data quality, reporting, and compensation administration. Build scalable processes, documentation, and employee-facing HR technology while contributing across a lean People & Culture team.

Provide onsite case management to formerly homeless individuals and families: maintain caseloads, complete housing eligibility and voucher paperwork, conduct home visits and monthly sessions, develop individualized service plans, connect clients to community resources, support independent living skills, enter HMIS notes within 48 hours, liaise with landlords and partner agencies, and assist with transportation for appointments and community activities.

Manage program data collection, quality, analysis, and reporting. Pull and automate reports from HMIS and internal databases, build and maintain Tableau dashboards, clean and document data pipelines, train staff on data entry and systems, and ensure compliance with grant and data privacy standards to support program performance and outcomes.
