Manage program data collection, quality, analysis, and reporting. Pull and automate reports from HMIS and internal databases, build and maintain Tableau dashboards, clean and document data pipelines, train staff on data entry and systems, and ensure compliance with grant and data privacy standards to support program performance and outcomes.

Provide intensive field-based case management for chronically homeless clients with severe mental health disorders. Manage caseloads, engage clients, link to housing and community resources, assist with subsidized housing applications, coordinate treatment plans with interdisciplinary FSP team, perform crisis assessment/intervention, maintain required documentation, and collaborate with partner agencies to support housing stability and independence.

Provide intensive, strengths-based case management to transition people experiencing homelessness into permanent scattered-site housing. Manage caseloads, conduct home visits and orientations, complete housing voucher paperwork and recertifications, coordinate community referrals and services, support independent living skills, enter notes in HMIS/CHAMP, and build relationships with landlords and partner agencies. Flexible hours and travel across multiple sites required.
