Manage consumer billing for private contracts: submit and track insurance claims, resolve denials/rejections, maintain consumer records, perform high-volume data entry, and provide invoice and reporting support to ensure timely reimbursement.

Oversee SCO and One Care contracts with six insurers; provide administrative and clinical supervision to four supervisors and ~20 care coordinators; ensure compliance through QA/QI activities; maintain collaborative relationships with external clinical and fiscal partners; provide leadership, direction, and continuous quality improvement for the program.

Perform comprehensive in-home assessments of older adults, document findings in an electronic statewide database, create care plans, coordinate services with vendor agencies, and communicate with families and community professionals as part of an interdisciplinary team.

Provide in-home case management for older adults and individuals with disabilities: conduct home visits and comprehensive assessments, develop person-centered care plans, document and run reports, and collaborate with an interdisciplinary team to support aging-in-place.

Provide onsite administrative support including data entry, phone/email handling, scanning, copying, filing, and office organization. Cover reception and other administrative staff as needed, support projects by compiling materials and running reports, and maintain supply/meeting room cleanliness.
