Responsibilities include assessments, individualized service plans, referrals, home visits, group facilitation, crisis intervention, documentation, HUD compliance, client engagement, and coordination with shelter, relapse prevention, sober housing, and community-service teams. The role requires case management, motivational interviewing, housing resource knowledge, data entry, driving, and schedule flexibility.

Provide case management to homeless individuals in early recovery, including assessments, service plans, daily support, referrals, advocacy, crisis intervention, documentation, and coordination with community providers.
