Provide case management for clients with mental illness and/or addiction: develop and implement treatment plans, coordinate transportation and community resources, maintain electronic charting, collaborate with treatment teams, assist in crisis intervention, conduct outreach, and ensure compliance with Medicaid/ODMHAS/CARF and HIPAA standards.

Supervises school-based behavioral health counselors and oversees clinical care, documentation, compliance, performance, training, billing utilization, and quality improvement. Builds relationships with schools and communities, supports program growth, resolves grievances, provides crisis expertise, and conducts diagnostic assessments and treatment planning for students with mental health or substance use needs.

Deliver pharmacy medications to patients' homes, maintain medication safety and temperature control, follow HIPAA and pharmacy handling procedures, provide courteous communication with patients/caregivers, document transactions, read maps/directions, and perform occasional lifting and physical tasks.

As a Psychiatrist at Signature Health, you'll deliver compassionate psychiatric care, conduct evaluations, and collaborate with healthcare professionals to enhance patient outcomes with a focus on mental health and addiction services.

Provide nursing care in outpatient behavioral health: intake vitals, med administration, specimen collection, documentation, provider coordination, prior authorizations, and patient follow-up while maintaining licensure and compliance.

The Case Manager develops and implements client treatment plans, provides transportation and community resources, and supports client engagement in treatment activities.

Supervise and lead community nursing team to ensure timely, patient-centered care; manage staff performance, training, scheduling, quality improvement, care coordination, and occasional direct patient care. Support clinical protocols, compliance, recruitment, and after-hours admissions.

The Licensed Practical Nurse will provide patient care, consult with providers on care plans, administer medications, and maintain patient records while ensuring quality and compliance with healthcare regulations.

Provide crisis intervention, de-escalation, risk screening, safety planning, and resource referrals via phone, chat, and text. Document encounters in the EHR, collaborate with emergency services and clinicians, maintain knowledge of mental health resources, and follow HIPAA and agency policies. Participate in training and supervision.
