Partner with families of children birth to three with developmental delays to coordinate ESIT services, assist with evaluations, develop and implement IFSPs, connect families to community resources, coordinate transitions, maintain documentation, and collaborate with multidisciplinary team members.

Provide job development, on-site training, and employer support to help individuals with developmental disabilities obtain and maintain community employment. Create individualized career plans, perform workplace assessments, coach participants and employers, maintain case records and schedules, submit electronic notes, and coordinate with referral agencies and families.

Assist individuals with disabilities to obtain, maintain, and advance community-paid employment. Conduct intake and discovery, develop individualized career plans, train participants and employers on job tasks and communication, provide on-site support and travel training, maintain case records and HIPAA-compliant notes, coordinate with counselors and community partners, and track schedules, hours, mileage, and outcomes.

Lead donor cultivation and community outreach to grow partnerships and fundraising. Represent the organization at events, manage donor CRM records, track engagement metrics, prepare acknowledgments and reports, and collaborate with program staff to translate needs into donor messaging.

Provide job discovery, placement, on-site training and follow-along support for individuals with disabilities; build employer relationships, develop individualized career plans, create video resumes, assess workplaces for safety and accessibility, and coordinate with case managers and vocational counselors to maintain successful employment outcomes.
