Teams at Seven Counties Services

Provides hospital security and patient transportation, responds to emergencies and calls using a therapeutic approach, monitors surveillance cameras, reports unusual occurrences, and prepares written incident reports. Full-time position with shift and weekend differentials.

Provides direct support to individuals with mental health and co-occurring disorders by teaching daily living skills, facilitating treatment objectives, leading psychoeducation and community activities, offering personal care and crisis assistance, supporting medication routines, providing transportation, documenting services, and collaborating with families and community providers. Requires strong communication, teaching, de-escalation, judgment, and caregiving skills.

Provides comprehensive case management for clients with developmental disabilities, including needs assessments, service planning, referrals, appointment tracking, progress monitoring, advocacy, documentation, provider consultation, family education, and crisis assistance. Coordinates emergency services and may serve as a Primary Case Coordinator. Requires compliance with regulatory standards, possible on-call availability, frequent travel, reliable transportation, and a current driver’s license.
