Provide and monitor person-centered support plans for ECF and residential program participants. Complete and maintain required documentation, coordinate and document meetings, train staff on individual plans, conduct unannounced visits, assist with job development (ACRE), and provide direct support to members as needed while ensuring compliance with licensure, Medicaid waiver, and agency policies.

Provide direct care and supervision to persons served at worksites, document daily notes in Timas, maintain records for billing, build employer relationships, consult with supervisors, and encourage appropriate workplace behavior and dress.

Provide and coordinate wellness and adapted recreational programs for persons supported (PS), track progress and outcomes, supervise staff during events, maintain equipment and paperwork, coordinate community activities and trips, train direct support staff, and ensure compliance with state guidelines and facility policies.

Implement and monitor person-centered support plans for day and residential programs, complete and maintain required paperwork, coordinate meetings and training, communicate with staff and external stakeholders, perform on-call duties and unannounced visits, and assist with hiring and staff orientation.
