Provide case management and housing-related services to residential clients, conduct assessments and quarterly service plans, maintain regular client contact, document assessments and progress notes in electronic records, and support clients' integration and independent living within the community.

Provide rehab assessments and develop strength-based psychosocial rehabilitation recovery plans. Deliver hands-on living-skills support, individual and group education, facilitate curricula, participate in multidisciplinary team meetings, complete documentation (assessments, recovery plans, progress notes, discharge summaries), and provide case management and housing-related services to residential clients.

Provide direct patient care as a full-time Staff Registered Nurse on Med-Surg Telemetry (day shift 7:00 AM–7:30 PM). Monitor telemetry patients, perform nursing assessments, administer treatments, and collaborate with the care team. Must hold NYS RN license and BLS/ALS certifications.
