Provide outreach, eligibility assessment, and case management to Veterans experiencing or at risk of homelessness. Create individualized housing plans, coordinate services and benefits, perform crisis intervention, maintain accurate records, and connect clients to community resources. Conduct community and home-based meetings and drive agency vehicle for outreach.

The HiFI Case Manager provides support to individuals experiencing chronic homelessness by helping them secure and maintain housing and access necessary services.
