Lead and support a home health clinical team: oversee patient services, guide assessment and care planning, ensure quality and regulatory-compliant clinical records, conduct staff performance evaluations, and provide clinical mentorship and direction to improve home health care delivery.

Coordinate and schedule home health aides, field new patient inquiries, log and mediate caregiver/client interactions, follow up on assignments, optimize authorized care hours (CICOA/WAIVER/VA/Prior Authorization), and participate in on-call rotation to support caregivers and clients.

Manage the complete referral process for home health clients: identify care needs, verify benefits, coordinate services, cultivate and expand referral relationships, ensure HIPAA/compliance, and support company growth. Requires ~1 year relevant experience and a valid driver's license.
