Provide skilled home health nursing as a Case Manager: perform assessments, make independent clinical decisions, manage caseloads, coordinate care, document visits, travel to patient homes, and assist with physical tasks (lifts up to 50 lbs). Maintain required licensure, valid driver’s license/transportation, and pass background checks.

Provide home-based skilled nursing as a Pay-Per-Visit PRN Registered Nurse Case Manager, completing Start of Care and recertification visits. Deliver autonomous, patient-centered care, perform physical tasks at patients' homes, document clinical findings, coordinate with care team, and maintain professional licensure and background clearance. Flexible scheduling with travel between client homes required.

Provide skilled home health nursing as a Registered Nurse Case Manager: manage caseloads, make autonomous clinical decisions, deliver one-on-one in-home care, perform physically active tasks, travel to patients' homes, and complete required documentation and background checks.
