Provide individual and family counseling to older adults in skilled nursing and long-term care settings. Coordinate care with facility teams, follow clinical guidelines and organizational protocols, educate staff, support holistic patient-centered care, and maintain timely documentation. The role requires independent practice, collaboration with clinical and administrative support teams, and a valid license as a psychologist, licensed clinical professional counselor, or clinical social worker in MD, DC, VA, or PA.

Prospects long-term care facility partners, builds relationships with healthcare decision-makers, generates qualified leads, schedules presentations, manages the partnership pipeline, records outreach in a CRM, monitors market opportunities, and supports transitions to account management and onboarding. The role requires healthcare business development experience, territory management, independent work, KPI achievement, and approximately 25%–75% travel across assigned markets.
