Provide nursing care within an intensive outpatient behavioral health program for older adults. Collaborate with interdisciplinary team, support community education/outreach, and contribute to compliance, program development, and quality improvement. Schedule is 32 hours/week, weekdays only.

Provide nursing care in an Intensive Outpatient Program for older adults: collaborate with psychiatric physician, social worker, and CNA; support patient care, community education/outreach, and program compliance/quality improvement. Full-time, Monday–Friday clinic role with no on-call or weekends.

Provide comprehensive nursing care for behavioral health patients as part of an interdisciplinary team. Conduct community outreach and educational presentations, support program compliance, participate in quality improvement, and collaborate on treatment planning with physicians, social workers, and CNAs.

Provide direct nursing care within a behavioral health program, collaborating with a psychiatric physician, social worker, and CNA. Deliver patient assessments, support treatment plans, participate in quality improvement, and conduct community education and outreach to promote mental health resources.

Lead community education and referral development for a geriatric intensive outpatient program. Develop and deliver training, build partnerships with hospitals and community organizations, design outreach and education plans, monitor referral performance, ensure regulatory compliance, and use data-driven marketing strategies to grow program engagement.
