Provides nursing care to older adults in an intensive outpatient behavioral health program. Collaborates with psychiatric physicians, social workers, and CNAs; supports patient treatment and progress monitoring; participates in quality improvement and compliance activities; and delivers community mental health education and outreach. The full-time schedule is Monday through Friday, 8 a.m. to 5 p.m., with no weekends or on-call duties.

The role is full time, Monday through Friday, with no weekends or on-call duties.

Leads an intensive outpatient geriatric behavioral health program, supervises a multidisciplinary team, builds community relationships, develops patient referrals, grows census, and ensures compliance with professional and ethical standards. The role includes community education, presentations, provider outreach, and coordination with hospitals and community resources. Monday-Friday schedule with no weekends or on-call duties.
