The CM Specialist-2 provides case management for individuals in the mental health system, coordinating team services, assessing needs, and assisting with independent living skills. Responsibilities include documentation, advocacy, and support during appointments while maintaining updates on community resources and participating in team meetings.

Coordinates Philadelphia’s Alternative Response Unit behavioral health program across fire, behavioral health, and criminal justice partners. Supervises crisis staff, ensures protocol compliance, documentation, follow-up, referrals, staffing, and quality improvement. Develops policies, training plans, budgets, and reports; maintains provider relationships; participates in interagency planning; and responds to challenging cases. The role includes field supervision, emergency-system work, occasional after-hours availability, and collaboration within a unified command structure.

Provides rapid outreach, assessment, service planning, benefits applications, and referrals for adults with recurring behavioral health crises. Conducts face-to-face engagement, investigates abuse or rights violations, coordinates community resources, advocates for clients, and links individuals to services within a 10–15-day response period. Maintains resource knowledge, documentation, communication availability, and collaboration with behavioral health agencies and treatment teams.

Manages employee complaints, grievances, investigations, discipline, corrective actions, performance improvement plans, and performance evaluations. Oversees HR compliance activities, background checks, HRIS duties, documentation, audits, licensing reviews, and accreditation support. Ensures adherence to employment laws and behavioral health regulations, identifies risks, develops policies, reports trends, and supervises HR personnel responsible for compliance-related processes.

Provide community-based case management for high-need adults with behavioral health and co-occurring conditions. Assess strengths/needs, develop and monitor person-centered plans, coordinate with multidisciplinary teams and external providers, assist with benefits, housing, vocational and forensic supports, accompany clients to appointments, provide training on life skills and public transit, complete documentation, participate in on-call rotation and weekend/evening shifts.

Provide administrative support for the Recovery House Initiative: manage referral intake, track data and census, assist with authorizations, licensing, HMIS/CEABRS adherence, reporting, meeting coordination, training tracking, and collaborate with internal and external partners.

Provide comprehensive assessments and evidence-based individual/group therapy (CBT, DBT, MI, IDDT, harm reduction) for high-need adults with co-occurring mental health and substance use disorders. Participate in multidisciplinary ACT team meetings, case management, community and forensic liaison, maintain documentation and compliance, attend trainings, and participate in on-call, evening and weekend shifts to support community tenure.

Design and produce interactive dashboards and visualizations (Tableau/Power BI) to communicate child-welfare data. Lead project teams, partner with analysts and IT, translate complex analyses for diverse stakeholders, and train staff on data communication and dashboard use.

The Nursing Manager supervises nurses and oversees clinical health services, coordinates activities, supports training, ensures quality service delivery, and manages department functions.
