The CM Specialist-2 provides case management for individuals in the mental health system, coordinating team services, assessing needs, and assisting with independent living skills. Responsibilities include documentation, advocacy, and support during appointments while maintaining updates on community resources and participating in team meetings.

Coordinates Philadelphia’s Alternative Response Unit behavioral health program across fire, behavioral health, and criminal justice partners. Supervises crisis staff, ensures protocol compliance, documentation, follow-up, referrals, staffing, and quality improvement. Develops policies, training plans, budgets, and reports; maintains provider relationships; participates in interagency planning; and responds to challenging cases. The role includes field supervision, emergency-system work, occasional after-hours availability, and collaboration within a unified command structure.

Provides rapid outreach, assessment, service planning, benefits applications, and referrals for adults with recurring behavioral health crises. Conducts face-to-face engagement, investigates abuse or rights violations, coordinates community resources, advocates for clients, and links individuals to services within a 10–15-day response period. Maintains resource knowledge, documentation, communication availability, and collaboration with behavioral health agencies and treatment teams.
