Provide person-centered case management for clients on CADI, BI, EW and DD waivers. Assess needs, develop and monitor service plans, coordinate resources and providers, attend interdisciplinary team meetings, and maintain case notes to generate billable units.

Provide person-centered case management for adults on CADI/DD waivers, including assessment, service planning, resource coordination, provider monitoring, interdisciplinary team meetings, and case noting. Caseload split between Hennepin and Scott Counties; travel between counties required. Maintain documentation and billable units.

Provide person-centered case management for clients on CADI, DD, or AC/EW waivers. Assess needs, develop and monitor service plans, coordinate resources and providers, attend in-person team meetings, and complete billable case notes. Work primarily in the six-county metro area with a hybrid schedule option after training.

Provide person-centered case management for clients on CADI, DD, BI and AC/EW waivers. Assess needs, develop service plans, coordinate resources and providers, monitor service delivery, complete billable case notes, attend in-person interdisciplinary meetings, and manage a variable caseload (typically 10–40 clients).

Provide CFSS consultation to Minnesota Health Care Programs participants: educate about CFSS, agency and budget models; help write, review, and submit service delivery plans; offer ongoing support; provide provider lists; document complaints; assist with surveys and plan changes as requested by DHS.
