Oversee service delivery for individuals with disabilities, coordinate schedules, train and supervise support specialists, complete service documentation, and ensure compliance with 245D requirements. The role also provides at least 20 hours weekly of direct, person-centered support involving community participation, health and safety, household management, adaptive skills, employment support, and related services. The manager serves as a primary contact for staff, individuals, families, and care teams.

Provides consultation and ongoing support to people receiving CFSS. Educates individuals about CFSS, agency and budget models; helps develop, review, and update service delivery plans; submits plans for approval; provides provider and FMS resources; responds to questions; completes reviews, documentation, surveys, complaint reporting, and culturally responsive services while managing an independent caseload.

Provide person-centered case management for adults on CADI/DD waivers, including assessment, service planning, resource coordination, provider monitoring, interdisciplinary team meetings, and case noting. Caseload split between Hennepin and Scott Counties; travel between counties required. Maintain documentation and billable units.
