Teams at People Incorporated Mental Health Services

Provide targeted case management for adults with chronic mental illness or health conditions: assess needs, develop person-centered support plans, coordinate referrals and services, maintain documentation, attend client meetings, and build long-term collaborative relationships while operating autonomously and meeting county onsite requirements.

Provide waiver case management for adults with chronic mental illness: develop person-centered Support Plans, authorize services, coordinate care, document interventions, maintain partner relationships, and meet clients twice annually. Hybrid schedule, manageable caseload (~30), and metro residency required.

Provide intake, initial engagement, barrier resolution, and short-term coordination to connect individuals to appropriate behavioral health services. Track referrals, document interactions in the EHR, support transitions to long-term care teams, collaborate with interdisciplinary partners, and participate in trainings and occasional on-call coverage.
